HOPEWELL HOME HEALTHCARE is sourcing candidates for an OCCUPATIONAL THERAPIST (PRN) for HOUSTON, TX area for geriatric population.

 

As an Occupational Therapist / OT you will:

  • Work in patients' homes as well as Agency office
  • Evaluate patient and establishes short and long-term goals based on the client's current level of functioning and potential for improvement.
  • Identify problems and establishes a Plan of Care in consultation with the client, physician, and clinical nursing team. Re-evaluate, observe, document, and report changes in patient condition and revise the Plan of Care as needed in accordance with the patient’s functional and cognitive needs.
  • Guides and instructs the client in therapy and self-care that are directed toward improving independence, maintaining strength, range of motion and fine motor coordination as well as physical and cognitive functioning.
  • Complete patient admissions and discharges.
  • Maintain open and ongoing communication to ensure that patient needs are met.

 

Credentials:

  • Must provide valid and current automobile liability insurance
  • Must provide a current CPR card
  • Graduate of an accredited school of Occupational Therapy.
  • State licensure as an Occupational Therapist.
  • Valid Driver’s License
  • Critical thinking skills, decisive judgment
  • Ability to perform work that requires frequent standing, bending, reaching, squatting, kneeling, moving, lifting of patients and/or equipment up to 50 pounds.
  • Experience with the geriatric population
